Ted was born February 15, 1981 at Grant Hospital, son of Sylvester Levi Forte Sr. and the late Tammy Forte.
On October 15, 2015, Ted was united in marriage to Melissa Anne Hopkins.
Surviving are a son Royale King Levi Forte and four daughters Tammy Nya Johnson, Amyah Joy Forte, Nya Venyce Forte and Phoenix Aunnamarie Forte. His sister Naomi Marie Forte and brother Drake Ian Forte, his loving grandmother Geraldine McGee, Uncles Washington Mac Mohammed, (Yvette Mohammed), Marcus Byron McGee, (Valerie Mcgee), Rodney Lee Leeper Jr, Pastor Victor Bowman, (Sherry Bowman), Wallace Brewster, (Elaine Brewster), Johnny Ray Allen (Vanessa Allen), John Johnson (Mary Johnson), Aunt Althea Parker, Aunt Hazel Forte and Aunt Charise Garrett. Closest friends: Maurice Howell, Adrian Collier, Jwan Garner, Carl Lewis, Eddie Anderson Sr, Keith Reid Sr and Joshua Williams.
Preceded in death are his mother Tammy Forte, grandfather Washington Mac McGee Sr, and grandmother

His memorial service will be held on Saturday, February 8 at 1 pm at Faith Tabernacle. 1204 St. Clair Avenue, Columbus, Ohio.
